BABY STEPS PROGRAM AT AUBURN HOSPITAL The Baby Steps Program provides timely access to a safe, high quality, ‘woman-centred’ outpatient antenatal service to improve the health, well-being and experience for mothers and families living within the Cumberland Local Government Area. The Baby Steps program includes the development of triaging guidelines, a discharge process, a referral management procedure and developing “Did Not Attend” (DNA) management procedures. The program has seen significant improvements in antenatal care for women with the following key achievements: • 50% increase in transfer of care (discharge rate) – baseline 19%, improving to 70% in November 2020 • 100% of referrals entered into the information patient management (iPM) system (baseline 4%) • Majority of women booking in by 18 weeks gestation (vs 24 weeks) • Gestational age at booking in <14 weeks increased from 4% to 17% by November 2020. DIALYSIS FREE-TIME THROUGH INCREMENTAL PERITONEAL DIALYSIS (INCRPD) The aim of the project was to ensure that from June 2019, 90% of new patients starting peritoneal dialysis (PD) within Western Renal Service (WRS) would be screened for incremental PD (IncrPD) and those found suitable would be offered this model of care. PD patients and families have to invest a substantial proportion of their time on a daily basis to undertake this life prolonging treatment. Minimising patient discomfort and anxiety must be an important goal of patient care, particularly when patients are vulnerable and unprepared to manage their dialysis. The program has achieved significant outcomes, including: • Saving up to two hours and 15 minutes per day on procedure time and up to 20 hrs/day of dialysate dwell times • Increased shared decision making with patients, as their needs and preferences are identified at the start of PD • Higher occasions of patient reviews to identify any signs of under-dialysis or risks that may compromise the longevity of PD undertaken. • Decreased ‘Did Not Attend’ rates • 95.2% of all new patient starting PD screened
