Multidisciplinary Outpatient Care for Heart Failure Project To improve timely access to patients with chronic heart failure (CHF), Blacktown and Mount Druitt Hospital’s Cardiology Service has developed a specialised multidisciplinary outpatient care service for people suffering from CHF. The team aimed to improve measurable patient outcomes including length of stay, readmission back into hospital and patient reported experience measures.

The model of care has seen consistent improvement in patient outcomes with a reduction in re-hospitalisation and length of stay rates. This initiative’s success prevents worsening conditions, maintains heart function, and minimises the need for extended hospital stays, achieved through addressing follow-up care barriers including medication, symptom awareness, psycho-social support, and lifestyle factors.
