General Manager and Service Director Awards

Integrated Value-Based Care– Multidisciplinary Outpatient Care for Heart Failure, Blacktown and Mount Druitt Hospitals

The General Manager and Service Director Awards recognise individuals, teams and departments that have made a difference in their team, department, unit, service or within the local community. There are five categories to be recognised for, including: Living Our Values, Compassion & Empathy; Safety; Achieving Goals and Outcomes. WSLHD Quality Awards 2022 The annual WSLHD Quality Awards recognise the work of our nurses, doctors, allied health professionals, support staff and researchers. The awards showcase the hard work and delivery of innovative health programs and services that positively touched the lives of our patients, carers, and the wider community. Some of the annual award winners and their projects will be submitted for further recognition at a state and national level as part of the NSW Health Innovation Awards, the NSW Premier’s Awards and the Australian Council on Healthcare Standards Quality Awards. Submissions are aligned to 16 categories, including awards chosen by the Chief Executive, Board Chair and Health Care Quality Committee Chair. Selected projects can be entered at a state and national level for future recognition.

This project successfully reduced the 28-day readmission rate for chronic heart failure patients to Blacktown Hospital’s Cardiology Department from 22% to 11%. Improvements were demonstrated in different aspects of value, including patient experience and the effectiveness and efficiency of care.

Health Research and Innovation Award– Hospital Care for Vulnerable Families, Blacktown and Mount Druitt Hospitals

Excellence in Aboriginal Healthcare– Aboriginal Health Hub–A COVID-19 Cultural Crisis, Integrated and Community Health This project strongly supported the WSLHD Aboriginal community during the COVID-19 pandemic to access culturally appropriate information, vaccinations, advocacy, and helped in ensuring compliance with COVID-19 restrictions, promoting overall wellness in the community.

This project improved healthcare access for children who experience significant social adversity. Since the commencement of this clinic, 110 patients have been treated, along with an additional 137 patients who have been referred and triaged.
