inTouch Residential Aged Care Facility Pathway There are approximately 4,300 emergency department (ED) presentations per year from people living in Residential Aged Care Facilities (RACFs). The Integration & Transformation team within ICH developed an inTouch RACF pathway to ED presentations, reduce hospital admission for RACF residents with low acuity conditions and reduce representations to ED. This pathway provided real-time alternate care pathways such as providing rapid referrals to Hospital in The Home (HiTH), outpatient clinics, mobile imaging, and consultation with the residents’ regular GP alongside WSLHD medical specialists.

WSLHD inTouch Team

Between 1 April 2022 and 31 March 2023, the inTouch RACF pathway received 3,131 referrals for residents living in 65 RACFs across WSLHD. The service achieved 981 emergency department avoidances over 12 months, resulting in an overall emergency department avoidance rate of 31% .

Aboriginal Go4Fun Program in Western Sydney The Centre for Population Health Healthy School Years team delivers the Go4Fun program, a healthy lifestyle program for children aged 7–13 years who are above a healthy weight and their families. The program is run by health professionals, including dietitians and exercise physiologists, and cultural support workers. Participating families learn about healthy lifestyle choices over the 10-week program run after school. In October 2022 the Healthy School Years team partnered with Kimberwalli (in Whalan) to deliver the Aboriginal Go4Fun program. Our partners at Kimberwalli told us: “The community wanted ideas for fresh, affordable family meals and getting active together”. For the parents, this program has provided a great opportunity to celebrate some healthy

lifestyle wins and to swap meal ideas. In 2022-23, 203 children completed the Go4Fun program.
